Job Details

Provide patient triage, nursing assessments, wound care, immunizations, and chronic disease support
Coordinate patient care, treatment plans, medication management, and follow‑up services
Participate in Enhanced Care Management and interdisciplinary meetings
Maintain vaccine inventory, medication logs, crash cart supplies, and clinic workflows
Deliver culturally competent education and care across a range of health topics and needs

Requirements
Associate or Bachelor’s degree in Nursing and a current California RN license
Two years of RN experience and current Basic Life Support certification
Strong communication skills and ability to thrive in an interdisciplinary setting
